The Cosmopolis School District processes new student enrollments online using the Skyward Family Access system.  The first step is to select “New Student Enrollment” and complete the short survey.  After you have completed the survey, you will be provided a Skyward Family Access account with instructions on how to complete the remainder of the enrollment process within 24 hours.

Please Note:  During the enrollment process, you will have the opportunity to scan and attach the required documentation to the application. If you do not have access to a scanner or a computer, you may schedule an appointment at the school to complete the process.  The following documentation is required to enroll a student:

  • Child’s Legal Name and Proof of age: Acceptable documents include, but are not limited to: birth certificate, passport, visa, Department of Social and Health Services (DSHS) Medicaid voucher, baptismal certificate, adoption decree, immunization record, hospital, or physician’s certificate. If you are unable to provide a document listed above, please contact your school office to review other alternative documents.
  • Child's immunization records: You will need to show proof that your child has been immunized against communicable diseases including measles, mumps, polio, etc. or provide a certificate of exemption signed by a licensed health care provider.  If your student is not current on the immunizations, he/she will not be permitted to attend school.
  • Proof of residency: You will need to provide proof of residency that you reside within the Cosmopolis School District boundaries.

Here are a few things parents should know.

  • Prior to September 1, children must be five years old to enter kindergarten and four years old to enter preschool.
  • Do you reside outside the Cosmopolis School District boundaries? If so, you are required to annually submit a the the Non Resident Choice Form for each child. You will need to fill out and submit the Non Resident Choice Form directly to the school’s office to be considered for enrollment.

Annual Student Information Update

An annual information update must be completed for all students currently enrolled in the Cosmopolis School District. The annual information update will open on August 10, 2020 for the 2020-2021 school year. All families must complete an annual update for all students prior to the first day of school.
